RESTART Long COVID Rehabilitation Programme

Restart Programme is a unique 12 week programme that has been designed for people suffering from Long COVID, or are at risk of developing COVID. The programme can be done virtually or in the centre and will involve sessions in The Live Well Centre gym, people will be supported throughout the programme by having mentor calls with one of the team.

You can self-refer by  downloading the Medical Questionnaire and the COVID Specific Questionnaire.

You’ll need to get it back to us, either by:

• emailing the form to livewellcentre@middlesbrough.gov.uk
• dropping the form off at the Live Well Centre (inside Dundas Shopping Centre, Middlesbrough, TS11HR)
